    *All applications must include two photographs; a link to a website, portfolio, or social media page is also appreciated.

    This event takes place on concrete. A single vendor space is equal to two parking spaces, approximately 15' x 18' in size. This option is subject to availability. Upon acceptance, your booth fee will be $80.

  10. Additional Information

    This is a rain or shine event. Your application fee is non-refundable. Vendors must provide own tent/s. Payment is due upon acceptance. All vendors will have access to one uncovered parking space in the lower lot. All other vehicles must be parked at an alternative location, to be determined closer to event time.

  11. The facts set forth above in my application for Registration Permit for Creek Chic Vendor Application are true and complete. I understand false statements shall be considered sufficient cause for denial and/or revocation. To the fullest extent permitted by laws and regulations, applicant shall identify and hold harmless the City of Merriam and event organizers of the Johnson County Park and Recreation District and its officers, employees and agents from and against all claims, suits, damages, costs, demands, losses and expenses, direct, indirect or consequential (including but not limited to fees and charges of attorneys and other professionals and court and arbitration costs) arising out of or resulting from the performance under this registration permit. The applicant is entirely and solely responsible for all acts while engaged in the operation of vending within the Creek Chic event.
  12. You should not, in any way, interpret this registration permit to mean that the
    City of Merriam, Johnson County Park & Recreation District, or any department, officer or employee thereof, endorses the applicant, the company, or the product being sold.
  13. Electronic Signature Agreement*

    By checking the "I agree" box below, you agree and acknowledge that 1) your application will not be signed in the sense of a traditional paper document, 2) by signing in this alternate manner, you authorize your electronic signature to be valid and binding upon you to the same force and effect as a handwritten signature, and 3) you may still be required to provide a traditional signature at a later date.
