Approved head coaches must submit the team info and special request form, team roster (if known), coach code of conduct, volunteer hold harmless form, background check form (required every two years), and concussion training certificate (required one time; if previously completed no need to complete again). Deadline to submit the forms for the fall league is July 5 for grades 1 - 8 and August 2 for the pee wee league.
Background Check Form (PDF) - Pee Wee & 1st - 4th Grade Coaches fax to 913-826-3414. All new coaches must complete; returning coaches will be done every two years.
Background Check for 5th - 8th Grade Coaches will be emailed instructions on how to complete.
Online Concussion Training - New coaches must complete this training and email copy of certificate to Jamie by July 31 for 1st -8th grade coaches and August 30 for pee wee coaches. Any returning coach who previously completed this does not need to complete again.
Equipment/Jersey Pick Up Date
All 1st - 8th grade coaches can stop by the picnic shelter at Heritage Soccer Park on Saturday, August 10 between 10:00 - 11:00 a.m. to pick up any equipment you need and jerseys your team ordered.
All pee wee coaches will pick up their jerseys at a later date.
