Soccer Coach Forms/Resources
Approved head coaches must submit the team info and special request form, team roster (if known), coach code of conduct, volunteer hold harmless form, background check form (required every two years), and concussion training certificate (required one time; if previously completed no need to complete again). Deadline to submit the forms for the league is July 21 for grades 1-8 and August 5 for the pee wee league.

- All coaches must complete a background check every two years. League coordinator will email this info to coaches as needed.

Equipment Pick Up Date
- All 1st - 8th grade coaches needing equipment for their team, will be emailed by mid-August on when and where those items can be picked up.
- Jersey pick up for pee wee league - 8th grade will be determined at a later date. An email will be sent out once that information is finalized.
- Shirts for the Little Surfers program will be picked up at their first session.
League Rules
Sportsmanship Committee
- The sportsmanship committee consists of various facility managers, sports league coordinators, and supervisor of officials within JCPRD.
- The sportsmanship committee works together when necessary to review any issues with behavior and codes of conduct during any JCPRD related event.
- The sportsmanship committee as a whole will decide on any disciplinary action as needed on a case-by-case basis.
